Zestard's "Multiple Custom Forms" extension allows admin to create multiple forms with custom form fields and generate shortcode to place those custom forms on multiple pages. After the form is filled from frontend by visitor, Email will be received to the recipient as set in Custom Form’s settings.

How to install/configure :
- Create Custom form by following Forms->Add New Form.
- Fill all required and necessary details.
- Add fields for custom form with add new field.
- Click on “Save Fields” to save form.
- Copy shortcode and paste it in any CMS page.
- Save CMS page or Static Block.
- Check on frontend for that CMS page.
- Fill up the form from frontend and Email will be received to the recipient as set in Custom Form’s settings.
